Can be faxed to 918-493-3315. We have accept the patient's demographics with the reason for referral. Physicians/clinicians may use our referral form (see link below). A physician/clinician letter explaining the patient's history is always welcome as it assists in evaluation. Only pertinent medical records are requested.
- We will notify your office when we schedule your patients.
- For urgent medical "work-in" appointments, the physician/clinician is requested to call Dr. Siegler to discuss.
