We request advanced payment for a new appointment, EEG or NEBA by cash, credit card, money order or cashier’s check.

After the appointment, we will provide a signed HCFA form so that you may submit for reimbursement from your insurance company.

Indian Nations as primary payer: your PCP must request and submit a purchase order to Child Neurology of Tulsa before appointments can be made.

Uninsured patients should contact our office for arrangements.
